Most Ohio’s Hospice LifeCare patients are served in their home, whether that is a private residence, an assisted living facility or a nursing facility. When asked, most people say that they want to remain in their homes until the end of their life, or at least as long as possible. Ohio’s Hospice LifeCare assists our patients with this every day. We have seen time and again that the support that Ohio’s Hospice LifeCare provides can assist individuals to stay home longer.

Hospice At Home

The support needed to keep a patient at home is different for each individual, but Ohio’s Hospice LifeCare has a full team of staff and volunteers to assist in meeting each patient’s unique needs. 

  • Nurses visit regularly to monitor medications and ensure that patients are comfortable and able to function at the highest level possible. 
  • Personal care specialists are available to assist with activities such as bathing and other personal care. 
  • Social workers visit to discuss what resources are available to help patients remain as independent as possible, to support caregivers so that they can care for their loved one longer, and to address concerns as they arise. 
  • Chaplains visit to provide emotional support to patients and their families, providing encouragement along the journey. 
  • Volunteers are available for a wide variety of services from companionship to assistance with medication delivery to occasional respite breaks for the caregiver. 
  • Other team members such as physicianspharmacists and dieticians provide recommendations to ensure that any barrier to a patient achieving his or her goal is removed.
  • Necessary medical equipment is provided by Ohio’s Hospice LifeCare to allow patients greater comfort and safety.
  • While hospice is not a 24-hour caregiving service, Ohio’s Hospice LifeCare staff members are only a phone call away to answer questions or provide guidance to a caregiver anytime of the day or night. After-hours staff members are available to make visits to the home if any concerns arise, even in the middle of the night.

If at any time, a patient’s needs exceed what is able to be provided at home, the Ohio’s Hospice LifeCare team will work with the patient and the family to explore options available. If the care of a facility is needed, Ohio’s Hospice LifeCare will continue to support the patient and family in the assisted living or nursing facility.
